OSHA Form 311/311A Report of Occupational Injuries and Illnesses Caused by Workplace Hazardous Materials In this form, the worker can describe the incident which caused the injuries or illness. OSHA considers occupational accidents to be injuries and illnesses that result from work where the employer should have known or should have been aware of the presence of material which can be hazardous to health. This form is useful in filing accident claims, and is available online at any OSHA office. What is the OSHA Form 301 report?

Monitoring labor safety is important for every industry.
